Can male partner treatment significantly reduce bacterial vaginosis recurrence?

high confidence  ·  Last reviewed May 20, 2026

Bacterial vaginosis (BV) is a common vaginal infection that often returns after initial antibiotic treatment. Many doctors and researchers have wondered if treating male sexual partners could stop this recurrence. Recent large studies suggest that treating the male partner does not significantly lower the rate of BV coming back in women.

What the research says

A 2024 systematic review analyzed six trials involving 953 couples. It found no significant reduction in BV recurrence when male partners were treated alongside the female partner compared to treating the female alone. The data showed a recurrence rate ratio of 0.97, meaning the outcomes were essentially the same 1.

Another 2021 randomized trial looked specifically at women with recurrent BV. It found that treating male partners with oral metronidazole did not significantly decrease recurrence rates overall. However, women whose male partners actually took the medication as prescribed were less likely to fail treatment. This suggests that partner adherence might matter more than the treatment itself 6.

Experts note that BV recurrence is complex and may involve factors beyond simple reinfection from partners. Current therapies have limitations, and new approaches like biotherapeutics or addressing the vaginal ecosystem are being studied to improve cure rates 237.

What to ask your doctor

  • Does treating my male partner make sense given my specific history of BV recurrence?
  • How important is it for my partner to take medication if we choose to treat him?
  • Are there newer treatments like vaginal biotherapeutics that might help prevent my BV from coming back?
  • What steps can we take together to improve adherence to any treatment plan we choose?
