Does acupuncture significantly improve cancer-related fatigue levels for patients?
Cancer-related fatigue (CRF) is a common and distressing side effect of cancer and its treatments. While exercise and activity pacing are often recommended, acupuncture has emerged as a potential complementary therapy. Recent meta-analyses suggest that acupuncture can significantly reduce fatigue levels compared to sham or usual care, though the overall quality of evidence is limited by small studies and risk of bias.
What the research says
A 2026 meta-analysis of 28 randomized controlled trials found that acupuncture significantly improved fatigue scores on the Piper Fatigue Scale compared to control groups (mean difference -0.56, 95% CI -0.74 to -0.38, P < 0.01) 36. This effect was consistent across different acupuncture types, with some modalities showing distinct advantages 3. Another network meta-analysis of 87 trials ranked warming needle as the most effective among 13 traditional Chinese medicine external therapies for CRF, followed by acupuncture and acupressure 4. Older evidence from a 2013 review of 7 studies also found that real acupuncture reduced fatigue more than sham acupuncture 5. However, the 2026 meta-analysis noted low overall study quality and risk of bias 36. A small 2007 trial of 47 patients reported significant improvements in general fatigue, physical fatigue, activity, and motivation after 2 weeks of acupuncture or acupressure 7. In contrast, activity pacing interventions showed only a small, non-significant effect overall, though a moderate benefit was seen in patients undergoing chemotherapy 1. Exercise also helps, with an optimal dose around 730 METs-min/week, but effects diminish at higher doses 2.
What to ask your doctor
- Could acupuncture be a helpful addition to my current fatigue management plan?
- Are there any acupuncturists you recommend who have experience with cancer patients?
- How many sessions might be needed to see an effect on fatigue?
- Should I continue other fatigue treatments like exercise or activity pacing while trying acupuncture?
- Are there any risks or side effects of acupuncture I should be aware of given my cancer type or treatment?
