Is [68Ga]Ga-FAPI PET imaging accurate for diagnosing Ovarian Cancer?

When diagnosing ovarian cancer, doctors often use imaging scans to find the primary tumor and see if the cancer has spread. [68Ga]Ga-FAPI PET is a newer type of scan that targets a protein called fibroblast activation protein (FAP), which is abundant in many cancers. Research shows that this scan is very accurate for detecting ovarian cancer, and in some cases it may be better than the standard FDG PET scan, especially for finding small tumor deposits in the abdomen and lymph nodes.

What the research says

A meta-analysis of 10 studies found that [68Ga]Ga-FAPI PET imaging has a high diagnostic odds ratio (DOR) of 48.69 for detecting primary tumors and 207.50 for detecting lymph node metastases in breast, ovarian, and cervical cancers, indicating excellent accuracy 6. In a direct comparison study of 41 patients with advanced epithelial ovarian cancer, Ga-FAPI-46 PET/CT showed higher sensitivity than FDG PET/CT for detecting peritoneal metastases and gave a higher peritoneal cancer index (PCI) score, which helps surgeons plan the extent of surgery 7. Another study of 30 patients compared [68Ga]Ga-FAPI-04 PET/MR with FDG PET/CT and found that the FAPI scan had advantages for detecting peritoneal metastases, especially around the diaphragm, and may better predict whether the tumor can be completely removed (R0 resection) 8. The same study noted that FAPI PET/MR had similar diagnostic accuracy to FDG PET/CT overall but was superior for certain metastatic sites 8.
