This Week in Cardiology: Atrial Fibrillation Management and Clinical Decision Support

Research regarding the management of atrial fibrillation (AF) featured prominently this week. In The New England Journal of Medicine, a randomized trial involving 207 patients with previously untreated persistent AF reported that pulsed field ablation (PFA) achieved a 56% treatment success rate at 12 months, compared to 30% for those treated with antiarrhythmic drugs [3].

A separate study published in Circulation journal: official journal of the Japanese Circulation Society provided a post hoc subanalysis of an RCT involving 341 patients with persistent AF and left atrial LVA ≥5 cm²; these researchers reported that complete low-voltage area (LVA) ablation showed no arrhythmia-free survival benefit compared to no LVA ablation [5].

Regarding clinical workflows and decision-making, a study in the Journal of the American College of Cardiology examined 490 patients with de novo left main or 3-vessel coronary artery disease. The authors found that real-time heart team decision making was noninferior to conventional face-to-face consultation regarding 1-year MACCE while potentially improving care efficiency [4].

Furthermore, a multicenter randomized trial published in NEJM Evidence involving 611 healthcare providers suggested that presenting diagnostic test information as interval likelihood ratios (ILRs) improved correct management decision rates by 11.2 to 18.3 percentage points for conditions such as pulmonary embolism and heart failure [2].

In a different area of clinical correlation, the Annals of medicine published a meta-analysis of cohort studies involving 3,100,610 participants. This analysis reported that endometriosis (EM) is associated with an increased risk of all-cause cardiovascular disease (CVD), specifically noting associations with myocardial infarction and coronary artery disease [1].
