This Week in Cardiology: Surgical Outcomes, Anticoagulation Risks, and Metabolic Factors

Research this week highlighted various risk factors influencing outcomes for patients with cardiovascular and metabolic conditions. In the journal Diabetologia, a post hoc analysis of a randomized controlled trial involving 2573 participants with type 2 diabetes and chronic kidney disease suggested that canagliflozin may mitigate increased cardiovascular risk associated with low-normal sodium intake while providing renal protection regardless of salt intake [1].

Moving toward surgical considerations, a meta-analysis published in Cardiology in review examined 35,863 patients with multivessel coronary artery disease and left ventricular dysfunction (ejection fraction ≤40%). The authors reported that off-pump CABG was associated with significantly reduced mortality risk and stroke risk in this specific patient population [3].

In a separate study regarding valve replacement, research in The American journal of cardiology indicated that mitral prosthesis-patient mismatch is associated with significantly higher all-cause mortality and heart failure hospitalizations following the procedure [5].

Regarding pharmacological management and long-term outcomes, a meta-analysis in Heart & lung: the journal of critical care examined 16852 adults with atrial fibrillation. The findings suggest that Factor XI/XIa inhibitors significantly reduced ISTH major bleeding (OR 0.30) and clinically relevant non-major bleeding (OR 0.47), but these inhibitors were associated with a significant increase in ischemic stroke risk compared to DOACs [4].

Finally, a meta-analysis in European journal of vascular and endovascular surgery: the official journal of the European Society for Vascular Surgery identified several factors linked to poststroke cognitive impairment, noting that age, gender, education level, and comorbidities such as hypertension and diabetes serve as key indicators for risk [2].
