This Week in Neurology: Neurodegenerative Diseases and Clinical Management of Stroke

This week’s research highlights diverse clinical considerations, beginning with acute stroke management. In JAMA, a study reported that intravenous tirofiban significantly improves 90-day functional outcomes for patients with acute ischemic stroke who experience an inadequate response to initial tenecteplase therapy [1].

Research regarding neurodegenerative conditions also featured prominently this week. A meta-analysis published in Revista clinica espanola identified a significant association between periodontal disease and Alzheimer's disease, reporting an odds ratio of 2.67 (95% CI: 1.41-5.06; p = 0.003) among over 290,000 participants [2].

In a separate study published in the Journal of Alzheimer's disease : JAD, a randomized clinical trial suggested that the MIND diet demonstrated superior longitudinal cognitive outcomes compared to a control diet over three years among older adults with elevated amyloid-beta and p-tau181 levels [3].

Other research focused on specific patient populations and underlying mechanisms. A narrative review in Frontiers in Medicine examined the impact of menopause and biological aging on women with multiple sclerosis, concluding that while symptoms may worsen in midlife due to overlapping factors, menopause does not appear to be a distinct clinical inflection point for disease activity or EDSS progression [4].

Finally, another narrative review in Frontiers in Medicine explored the molecular mechanisms between sudden unexpected death in epilepsy (SUDEP) and long QT syndrome (LQTS), where authors proposed that cardio-cerebral ion channel dysfunction may link these conditions, suggesting clinicians consider cardiac safety of ASMs and genetic risk in relevant patients [5].
