This Week in Neurology: Parkinson Disease Biomarkers and Stroke Management Strategies

Research regarding Parkinson disease this week focused on both anatomical markers and pharmacological interventions. In the Journal of neurology, a meta-analysis reported significant reductions in the cross-sectional area of the vagus nerve in patients with Parkinson's disease compared to controls [1].

These findings suggest that such measurements could serve as a potential anatomical biomarker for the condition [1].

Conversely, research published in Neurological sciences: official journal of the Italian Neurological Society and of the Italian Society of Clinical Neurophysiology examined the role of GLP-1 receptor agonists. This meta-analysis reported that these agonists provided no significant motor or non-motor benefits for patients with idiopathic Parkinson's disease and noted associated risks such as weight loss [2].

Regarding stroke management, a meta-analysis published in Neurology evaluated the impact of anesthesia during endovascular thrombectomy for ischemic stroke. The study, which included 1601 patients, found that general anesthesia was associated with a higher odds ratio of 1.24 for functional independence at 90 days compared to non-GA techniques [3].

A separate study in the European stroke journal provided a post-hoc analysis of a randomized controlled trial involving 832 patients with acute ischaemic noncardioembolic stroke. The authors reported that early tirofiban improved functional outcomes specifically in patients without diabetes, while showing minimal difference in patients with diabetes [5].

Additionally, the European stroke journal published a Phase 3 randomized controlled trial protocol evaluating intra-arterial thrombolysis with alteplase for patients with acute ischaemic stroke due to anterior circulation large-vessel occlusion; however, as this is a protocol publication, no clinical outcomes or safety data are currently available [4].
