This Week in Oncology: Muscle Loss, HCC Treatment, Breast Radiotherapy

From the New England Journal of Medicine perspective on supportive care issues, a systematic review and meta-analysis reported that systemic cancer treatment is associated with a small but significant decline in skeletal muscle mass [1].

The authors describe an average loss of approximately 5% over a period of 90 days across studies involving more than 10,000 patients. These findings suggest clinicians might consider monitoring skeletal muscle mass in patients receiving ongoing systemic therapy.

Meanwhile, research focused on liver cancer appeared in Liver international : official journal of the International Association for the Study of the Liver [2].

A systematic review and meta-analysis evaluated first-line treatment options for unresectable hepatocellular carcinoma specifically within East Asian populations. The study results indicate that lenvatinib combined with a PD-1 inhibitor may offer superior survival outcomes compared to bevacizumab plus PD-1 inhibitor, based on available retrospective data.

Elsewhere this week, attention turned to breast cancer radiotherapy schedules in The Lancet Oncology [3].

A phase 3 randomised trial involving 4087 patients with early breast cancer compared two hypofractionated regimens against a standard approach. At ten years follow-up, the researchers found that delivering 26 Gy in five fractions was non-inferior to administering 40 Gy in fifteen fractions regarding recurrence rates and toxicity profiles.

A separate study published in American journal of clinical oncology examined immunotherapy applications for hormone receptor-positive breast cancer [4].

The pooled analysis revealed that while complete response rates improved, overall response benefits were not consistently observed. Consequently, the authors suggest weighing potential risks against limited gains when considering this approach for HR+ disease.

Finally, Supportive care in cancer : official journal of the Multinational Association of Supportive Care in Cancer highlighted quality-of-life considerations [5].

A systematic review and meta-analysis spanning 18 countries included over 20,000 lung cancer patients. Lower pre-treatment global quality of life was associated with a seven percent increased mortality risk, though researchers caution against inferring direct causality from these observational associations.
