This Week in Oncology: Advancements in Lymphoma Therapies and Diagnostic Techniques

Recent research highlights diverse approaches to managing hematologic malignancies, particularly within the context of follicular lymphoma and diffuse large B-cell lymphoma (DLBCL). In Blood, a Phase 3 randomized controlled trial involving 1030 patients with previously untreated advanced follicular lymphoma reported that the combination of rituximab and lenalidomide (R2) provided median progression-free survival comparable to rituximab-based immunochemotherapy [2].

This suggests that R2 may serve as a viable chemotherapy-free alternative for these patients. Similarly, in Lancet (London, England), a Phase 3 randomized, double-blind, placebo-controlled trial evaluated the addition of tafasitamab plus lenalidomide to R-CHOP for patients with high-risk DLBCL or HGBL. The study found a statistically significant improvement in progression-free survival compared to R-CHOP alone, although it noted higher rates of severe adverse events [3].

Another study published in Blood examined chronic lymphocytic leukemia (CLL) specifically in patients without TP53 aberrations. This Phase 3 randomized controlled trial of 926 patients found that venetoclax-based regimens—specifically RV, GV, and GIV—provided superior progression-free survival compared to chemoimmunotherapy [5].

The researchers noted that these venetoclax-based combinations offered superior 5-year progression-free survival in this specific patient population.

Moving toward solid tumor research, a study in Lancet (London, England) reported on the management of PD-L1-positive gastric adenocarcinoma. The authors described how neoadjuvant serplulimab combined with SOX chemotherapy significantly improved event-free survival and safety profiles for these patients [4].

Additionally, regarding diagnostic procedures, a randomized clinical trial published in JAMA evaluated transbronchial biopsy techniques across 500 patients at 9 US centers. The study reported that a 1.1-mm cryoprobe yielded an 88.6% diagnosis rate compared to 78.8% with 2.0-mm forceps, suggesting the smaller cryoprobe may provide higher diagnostic yields for lung nodules and transplant cases [1].
