Can genetic testing in Pakistan help understand treatment-resistant depression?

moderate confidence  ·  Last reviewed May 12, 2026

Treatment-resistant depression (TRD) means depression that does not improve after trying at least two different antidepressants. Genetic testing looks at your DNA to see how your body processes medications. In Pakistan, a large study called DIVERGE found that certain gene variants, especially in the CYP2C19 gene, are linked to TRD 3. This means testing could help doctors choose antidepressants that are more likely to work for you.

What the research says

A major study in Pakistan (the DIVERGE study) analyzed over 3,600 patients with major depressive disorder. It found that genetic differences in the CYP2C19 enzyme, which helps break down many antidepressants, are associated with treatment-resistant depression 3. This suggests that genetic testing could identify people who may not respond to standard antidepressants because of how their body processes these drugs 3.

Other research on TRD treatments, such as deep brain stimulation 1, psychotherapy 2, esketamine 58, transcranial magnetic stimulation 6, and ketogenic diet 7, does not directly address genetic testing in Pakistan. However, these studies show that TRD is a complex condition with many possible causes, and genetic factors are one piece of the puzzle 34.

Ceramides, a type of fat in the blood, have also been linked to depression severity and treatment resistance 4. While not yet used in routine genetic testing, this highlights that biological markers may help personalize TRD treatment in the future 4.

What to ask your doctor

  • Is genetic testing for CYP2C19 available in Pakistan, and could it help guide my antidepressant choice?
  • What are the costs and limitations of genetic testing for treatment-resistant depression?
  • How would the results of a genetic test change my treatment plan?
  • Are there other biological tests, like blood markers, that might be useful for my depression?
  • Can you refer me to a specialist or clinic that offers pharmacogenetic testing for depression?
