Can machine learning predict anastomotic leaks in gastric cancer patients?

high confidence  ·  Last reviewed June 18, 2026

Anastomotic leak is a serious complication after gastric cancer surgery, occurring in 2% to 15% of patients and carrying high mortality. Machine learning (ML) models use patient data to estimate leak risk, potentially allowing earlier detection and prevention. Research shows ML models achieve good predictive performance, with some reaching AUC values above 0.85, particularly when they include postoperative lab results and immunonutritional markers.

What the research says

A systematic review of 18 studies (15,783 patients) found that ML models had a pooled external validation AUC of 0.795, meaning they correctly identified leak risk about 80% of the time 2. Models that included postoperative indicators (like lab values after surgery) performed significantly better than those using only preoperative data (AUC 0.843 vs. 0.748) 2. Another study using five ML models found that an immunonutritional index (combining immune and nutritional markers) outperformed inflammatory or nutritional markers alone, with an AUC of 0.87 8. A large study of 1,588 patients developed a LASSO-Logistic model that achieved an external validation AUC of 0.871, with a negative predictive value of 96.9% — meaning it could reliably rule out leak risk 9. Deep learning models analyzing intraoperative images also showed promise: ResNet18 achieved recall of 0.85 for duodenal stump leaks and 0.80 for esophagojejunal anastomosis leaks 10. However, internal validation often overestimates performance compared to external validation, suggesting some models may not generalize as well to new patient groups 2.

What to ask your doctor

  • Does your hospital use any machine learning tools to predict anastomotic leak risk after gastric cancer surgery?
  • What postoperative indicators (like CRP or albumin levels) are most important for assessing my leak risk?
  • How would knowing my predicted leak risk change my surgical or postoperative care plan?
  • Are there any ongoing studies or clinical trials using machine learning for leak prediction that I might be eligible for?
  • What is the current standard approach to monitoring for anastomotic leak after my surgery?
