Gastric Cancer: what the trials found

Several immunotherapies demonstrated significant improvements in survival outcomes for patients with gastric cancer. Pembrolizumab showed statistically significant improvements in Overall Survival (OS) across multiple trials, including those involving specific biomarkers like PD-L1 CPS ≥ 10 [6, 19] and in combination with chemotherapy 11. Specifically, Pembrolizumab demonstrated a median OS of 15.7 months vs 11.8 months in patients with PD-1 CPS ≥ 10 6, and 12.0 months vs 5.3 months in the same cohort 19. In trials involving Nivolumab plus chemotherapy, significant improvements were noted in both OS (14.39 vs 11.10; p<0.0001) and Progression Free Survival (PFS) (7.69 vs 6.05; p<0.0001) for patients with PD-L1 CPS ≥ 5 15. Tislelizumab also showed significant improvements in OS for both PD-L1 positive participants 21 and the overall intent-to-treat population 21.

Other targeted and systemic therapies showed varying results. TAS-102 demonstrated a statistically significant improvement in Overall Survival (5.7 vs 3.6; p=0.0003) 7, while TAS-118 plus Oxaliplatin significantly improved OS, PFS, and Time to Treatment Failure compared to standard treatments 18. In contrast, Avelumab did not show statistically significant improvements in OS [8, 14] or other primary endpoints like ORR 8. Docetaxel (XRP6976) showed a statistically significant improvement in the percentage of participants with 3-year Progression-Free Survival (PFS) 10, while Ramucirumab demonstrated an Objective Response Rate (ORR) of 41.1% vs 36.4% 13.

Clinical outcomes for other interventions were less definitive or did not reach statistical significance in primary endpoints. Olaparib did not show a statistically significant difference in OS 12 or PFS 12, and the results for Objective Response were also not statistically significant 12. 5-Fluorouracil showed a non-significant trend in Overall Survival (p=0.0565) 9. Early palliative care did not result in a statistically significant change in Quality of Life scores at 12 or 24 weeks 16.
