Can pentoxifylline improve kidney function and reduce inflammation for my disease?

Pentoxifylline is a medication originally used for blood flow problems. Recent research suggests it may also help people with chronic kidney disease (CKD). Studies show that pentoxifylline can modestly improve kidney function, measured by estimated glomerular filtration rate (eGFR), and reduce markers of inflammation like C-reactive protein (CRP) and tumor necrosis factor-alpha (TNF-α). What the research says

A 2026 meta-analysis of 19 randomized controlled trials involving 1,166 patients with CKD found that pentoxifylline significantly increased eGFR by about 4.59 mL/min/1.73 m² compared to control 310. The same analysis showed that pentoxifylline reduced urinary albumin excretion (a marker of kidney damage) and lowered inflammatory markers: CRP decreased by a moderate amount, and TNF-α levels also dropped 310. These findings suggest pentoxifylline has both kidney-protective and anti-inflammatory effects.

Earlier research supports these results. A 2004 review noted that pentoxifylline has anti-inflammatory and anti-fibrotic properties in animal and human studies, and it reduced proteinuria in patients with diabetic kidney disease and other forms of CKD 9. A 2015 meta-analysis focusing on anemia found that pentoxifylline increased hemoglobin levels in some studies, though the effect on anemia was not consistent across all trials 11.

It is important to note that the improvements in kidney function are modest, and the studies were relatively small. Pentoxifylline is not a standard treatment for CKD, but it may be considered as an add-on therapy for some patients, especially those with persistent inflammation or proteinuria despite standard care.
