Can text-message cognitive-behavioral therapy help reduce symptoms of major depressive disorder in young adults?

high confidence  ·  Last reviewed May 9, 2026

Text-message delivered cognitive behavioral therapy (CBT-txt) is a promising digital treatment for young adults with major depressive disorder. Several randomized clinical trials show that CBT-txt can significantly reduce depressive symptoms and improve functioning, with effects comparable to in-person therapy in some cases. This approach is especially useful for young adults, who often face barriers to traditional care.

What the research says

A 2023 randomized clinical trial of 103 young adults found that CBT-txt significantly reduced depressive symptoms compared to a waitlist control, with a large effect size (Cohen's d = 1.26) for anxiety symptoms and 41.3% of the treatment group achieving minimal or no anxiety symptoms at 3 months 6. The same trial also showed that CBT-txt increased behavioral activation and reduced perseverative thinking, which in turn reduced depression 9. A pilot randomized trial of 102 young adults found that CBT-txt participants were three times more likely to have minimal or mild depression at 2 months compared to controls, with the strongest effects seen in those with severe depression 10. Another study tested a fully automated text-based behavioral activation intervention (a core component of CBT) and found it significantly reduced depressive symptoms and anhedonia in adults with moderate depression 11. While these studies focus on text-based CBT, broader research supports CBT for depression: a trial of 265 patients found that CBT reduced avoidant personality traits and coping in major depressive disorder 3, and transdiagnostic behavior therapy showed comparable efficacy to disorder-specific therapies for depression in veterans 1.

What to ask your doctor

  • Could text-message CBT be a good option for me or my child?
  • How does CBT-txt compare with other treatments I am considering?
  • Are there any studies or programs offering CBT-txt that you would recommend?
  • Should I continue my current treatment while trying CBT-txt?
  • What signs should I watch for to know if CBT-txt is working?
