Can transcutaneous tibial nerve stimulation reduce urgency in older women with overactive bladder?

high confidence  ·  Last reviewed May 21, 2026

Transcutaneous tibial nerve stimulation (TTNS) is a non-invasive treatment that uses a small device to send mild electrical pulses to the tibial nerve. Research shows that adding TTNS to standard behavioral therapy helps older women with overactive bladder reduce urgency and nighttime urination more effectively than behavioral therapy alone. This approach is considered safe and well-tolerated.

What the research says

A randomized trial specifically in older women found that the group receiving TTNS plus behavioral therapy showed significant reductions in urgency and nocturia. The group receiving only behavioral therapy improved, but the addition of TTNS led to greater improvements in these specific symptoms 2.

Another study comparing TTNS to vaginal electrical stimulation found that both methods significantly reduced urgency episodes. However, TTNS maintained these improvements for a longer period, one month after treatment ended, whereas the other method did not sustain the same level of benefit 4.

A third study comparing TTNS to a different type of electrical stimulation in elderly women showed that TTNS reduced the number of urgency episodes and urge urinary incontinence. The other stimulation method only reduced nighttime bathroom trips, meaning TTNS addressed a broader range of symptoms 6.

What to ask your doctor

  • How does adding TTNS to my current behavioral therapy plan compare to taking medication for my urgency?
  • Is TTNS a suitable option for my specific symptoms, such as frequent nighttime urination?
  • What is the typical schedule for TTNS sessions, and how long do I need to use the device?
  • Are there any specific side effects I should watch for while using TTNS at home?
