Do GLP-1 receptor agonists reduce limb complications for people diagnosed with Peripheral Artery Disease?

Peripheral artery disease (PAD) narrows leg arteries, raising risks of pain, poor wound healing, and limb loss. For people with type 2 diabetes and PAD, GLP-1 receptor agonists (a class of diabetes and weight-loss drugs) appear to lower the chance of serious limb complications. Two recent meta-analyses show these drugs reduce major adverse limb events (MALE) — such as critical limb ischemia, revascularization procedures, and amputation — along with heart attacks and death.

What the research says

A 2026 meta-analysis of 6 randomized trials including 7,645 participants with type 2 diabetes and PAD found that GLP-1 receptor agonists significantly reduced major adverse cardiovascular events (MACE) by 14% (risk ratio 0.86) and all-cause mortality by 17% (risk ratio 0.83). However, the reduction in major adverse limb events (MALE) did not reach statistical significance in that analysis 9. Another 2026 meta-analysis of 10 studies with over 350,000 PAD patients reported a 34% lower odds of MALE (odds ratio 0.66, p=0.05), along with significant reductions in all-cause mortality (45% lower odds), MACE (32% lower odds), and myocardial infarction (32% lower odds) 10. A 2022 real-world study from Taiwan comparing GLP-1 receptor agonists to SGLT2 inhibitors in people with diabetes found no significant difference in MALE between the two drug classes, but both were associated with lower limb event rates compared to no treatment 11. The cardiovascular benefits of GLP-1 receptor agonists are thought to come from direct effects on blood vessel inflammation, weight loss, and blood sugar control 2. While the evidence is promising, the studies are observational or meta-analyses of trials not primarily designed for PAD, so results should be interpreted with caution.
