Do GLP-1RAs help improve walking distance for those with diabetes and Peripheral Artery Disease?
Peripheral artery disease (PAD) narrows leg arteries, causing pain and limited walking distance. For people with type 2 diabetes, PAD is more common and severe. GLP-1 receptor agonists (GLP-1RAs) are diabetes drugs that also protect the heart and blood vessels. A 2025 systematic review and meta-analysis directly tested whether these drugs help people with both conditions walk farther.
What the research says
A 2025 meta-analysis pooled data from 7 studies (3 randomized trials, 4 cohort studies) involving over 107,000 participants with type 2 diabetes and PAD 9. The analysis of two randomized trials with 847 participants found that liraglutide or semaglutide significantly improved walking distance (hazard ratio 1.10, 95% CI 1.05–1.15, p<0.001), with no variation between studies and moderate certainty of evidence 9. This means people taking these drugs were about 10% more likely to walk a given distance without stopping. However, the same meta-analysis found that GLP-1RAs did not significantly reduce the risk of major limb events (like amputation) in two other trials with 3,592 participants 9. A separate review notes that GLP-1RAs provide cardiovascular benefits across multiple scenarios, including symptomatic PAD, through direct effects on blood vessels and indirect effects from weight loss and blood pressure reduction 4. The weight loss effect of GLP-1RAs is about 39% less in people with type 2 diabetes compared to those without diabetes 1, which may partly explain why walking improvements are modest.
