Do self-help mindfulness and CBT reduce anxiety in distressed students?

high confidence  ·  Last reviewed May 9, 2026

For distressed university students, self-help mindfulness and cognitive behavioral therapy (CBT) delivered online can significantly reduce anxiety. A 2026 randomized controlled trial found that both a self-help mindfulness intervention and a combined mindfulness-plus-CBT program were more effective than no treatment for lowering anxiety symptoms in students with moderate to severe emotional distress 69. Importantly, adding CBT to mindfulness did not produce better results than mindfulness alone 69.

What the research says

A 2026 randomized controlled trial involving 335 university students from multiple Chinese universities compared a self-help mindfulness intervention (sMBI), a combined mindfulness and CBT intervention (sMBI+CBT), and a waitlist control group 69. Both active interventions were delivered via an internet-based website over 60 days 69. The results showed that both sMBI and sMBI+CBT significantly reduced anxiety compared to the control group, with effects lasting at least six months 69. However, there was no significant difference between the two active groups, meaning adding CBT did not improve anxiety outcomes beyond mindfulness alone 69. The study also found that mindfulness worked by increasing mindfulness skills and psychological flexibility, while the combined intervention worked through mindfulness and reduced cognitive reactivity 69.

Another 2025 study tested a digital CBT-based intervention in 390 Chinese high school students and found it reduced anxiety scores by about 2.5 points on the GAD-7 scale compared to controls 7. This supports the idea that digital self-help programs can be effective for younger students as well 7.

A separate 2025 systematic review of digital technology interventions for older adults also found significant anxiety reduction, though the evidence was low-certainty 8. While not specific to students, it suggests digital mental health tools can help across age groups 8.

Overall, the strongest evidence for distressed students comes from the 2026 trial, which directly tested self-help mindfulness and CBT in this population 69. The findings indicate that both approaches are effective, and mindfulness alone may be sufficient for anxiety reduction 69.

What to ask your doctor

  • Would a self-guided online mindfulness program be a good option for my anxiety?
  • How do I choose between a mindfulness-only program and one that also includes CBT?
  • Are there any risks or downsides to using self-help digital interventions instead of in-person therapy?
  • How can I tell if a self-help program is evidence-based and safe to use?
  • Should I combine self-help with other treatments, such as medication or counseling?
