What the research says
A 2022 meta-analysis of 23 studies (530 patients) found that active blood product removal strategies, such as extraventricular drains, thrombolytics, drainage-irrigation-fibrinolytic therapy, and neuroendoscopic lavage, reduced the rate of conversion to a permanent ventriculoperitoneal shunt 6. A 2026 meta-analysis of 6 studies (256 patients) directly compared blood product removal (BPR) to temporary CSF diversion (TCD) alone. BPR was associated with a 33% lower risk of permanent shunting (RR 0.67, p=0.047) 17. The same study also found that TCD alone had higher rates of CSF-related infections (26.54% vs 2.40%) and mortality (19.27% vs 3.47%) 17. Another review of ventriculosubgaleal shunts, a type of temporary diversion, reported a 9% infection rate and that most infants eventually needed a permanent shunt 3. Overall, the evidence suggests that actively removing blood products reduces the need for permanent shunting and may improve other outcomes.
