Does using imaging guidance during stenting lower heart attack risk for older patients with coronary artery disease?

What the research says
A 2024 meta-analysis pooled data from 9 randomized controlled trials involving 7,164 older adults (age 65+) who underwent PCI. It found that using intravascular imaging (IVUS or OCT) to guide the procedure significantly reduced MACE compared with angiography alone (relative risk 0.66, 95% CI 0.56-0.77, p<0.001) 6. The benefit was seen with both IVUS (RR 0.55) and OCT (RR 0.70) 6.
Other recent trials support these findings. The OCCUPI trial (2024) in patients with complex lesions showed that OCT-guided PCI reduced the 1-year rate of MACE (5.2% vs 8.5%) compared with angiography guidance 10. Similarly, the ECLIPSE trial (2025) in patients with severely calcified lesions found that intravascular imaging guidance lowered the 1-year target vessel failure rate (9.3% vs 13.2%) 11. The ULTIMATE Ⅲ trial (2024) also showed that IVUS-guided drug-coated balloon angioplasty improved vessel outcomes in high-bleeding-risk patients 9.
Importantly, the meta-analysis found no significant heterogeneity, meaning the benefit was consistent across studies 6. The evidence is strongest for IVUS, but OCT also showed a clear advantage. These results suggest that imaging guidance helps ensure optimal stent expansion and placement, reducing the chance of future blockages or heart attacks.
