Is DNA methylation testing accurate for identifying Cervical Intraepithelial Neoplasia?
DNA methylation testing is a newer method to help decide which women with a positive HPV test need a colposcopy (a closer look at the cervix). The test looks for chemical changes on certain genes that are more common in precancerous cells. Research shows that several methylation markers can detect high-grade cervical intraepithelial neoplasia (CIN2 or worse) with high accuracy, often outperforming standard Pap tests.
What the research says
A large meta-analysis of 51 studies in HPV-positive women found that the markers JAM3, C13ORF18, and PAX1 had the highest diagnostic odds ratios for detecting CIN2 or worse, meaning they were very good at distinguishing disease from no disease 1. In a separate study of women infected with HPV types 16 or 18, a test combining PAX1 and JAM3 methylation (called CISCER) showed 89.0% sensitivity and 95.3% specificity for CIN2+, with an area under the curve of 0.921 6. Another study validated a four-gene methylation score (CA10, DPP10, FMN2, HAS1) that accurately predicted cervical cancer, with an area under the curve of 0.995 in the training set and 0.934 in the validation set 7. The WID-CIN test, which analyzes methylation across the genome, also showed promise for identifying women with CIN3+ and those at risk of developing it in the next 1-4 years 5. Overall, DNA methylation tests are more objective and reproducible than cytology, and they can reduce unnecessary colposcopies while catching most cases of high-grade CIN.
