Danshen (Salvia miltiorrhiza) is a Chinese medicinal herb that has been studied for its potential to treat spinal cord injury (SCI). In animal models, researchers have tested whether Danshen extract can help repair damage and improve recovery. The evidence from multiple studies shows that Danshen extract can improve movement, reduce swelling, and protect nerve cells in rats with SCI. However, these findings are from animal studies only, and it is not yet known if they apply to humans.

What the research says

A systematic review and meta-analysis of 35 randomized controlled trials in animal models found that Danshen extract significantly improved motor function after SCI, as measured by the BBB score (a scale for movement recovery). The improvement was seen at 3, 7, 14, and 21 days after injury, with a standardized mean difference (SMD) of 4.53 at 3 days (p < 0.001) 410. The same analysis showed that Danshen reduced inflammatory markers such as TNF-α and IL-1β, decreased oxidative stress (measured by MDA), and lowered levels of the apoptotic marker Caspase-3, while increasing antioxidant activity (SOD) 410. It also significantly reduced spinal cord edema, with a SMD of -3.88 (p < 0.00001) 410. Subgroup analysis indicated that higher doses (>20 mg/kg) and water-soluble phenolic acids provided the best outcomes, with the most significant improvements seen in contusion and ischemic injury models 410.

A separate rat study using a weight-drop model confirmed that Danshen extract (given by oral gavage at 12.5 g/kg) significantly improved motor function recovery and reduced histopathological damage 9. This study also found that Danshen increased expression of neurofilament 200 (NF-H) and brain-derived neurotrophic factor (BDNF), which are important for nerve repair, and reduced markers of inflammation (CD11b) 9. Another pharmacokinetic study in SCI rats showed that when Danshen was combined with another herb (huangqin), the absorption of certain active compounds (phenolic acids) changed in ways that may be beneficial for treating SCI 11.
