This Week in Diabetes & Endocrinology: Weight Management Strategies and Metabolic Outcomes

This week’s research highlights various pharmacological and traditional approaches to managing weight and cardiovascular risk. In the Journal of the American College of Cardiology, a randomized controlled trial involving 27,564 patients with stable atherosclerotic cardiovascular disease found that evolocumab significantly reduced the risk of MACE [1].

The study noted that this reduction was particularly pronounced in patients with a BMI of 35 kg/m² or greater, suggesting a progressively larger relative risk reduction as BMI increased [1].

Moving to weight management specifically, a Phase 3b randomized controlled trial published in the Lancet (London, England) evaluated tirzepatide in 378 adults with obesity over 112 weeks [2].

The results indicated that patients receiving the maximum tolerated dose achieved a 21.9% reduction in body weight [2].

The authors suggest that while the maximum tolerated dose is effective for sustained weight loss, a 5 mg dose may serve as an alternative if the higher dose is poorly tolerated [2].

Additionally, a meta-analysis published in medRxiv explored Traditional Chinese Medicine (TCM) and found it was associated with a body weight reduction of 5.86 kg and a BMI reduction of 2.82 kg/m² compared to placebo or conventional treatments [4].

Other research focused on the broader clinical landscape and metabolic markers. A meta-analysis in PloS one reported a high prevalence of both microvascular and macrovascular complications, such as neuropathy and retinopathy, among adult Ugandans with diabetes [3].

Furthermore, a meta-analysis published in the American journal of physiology. Endocrinology and metabolism examined 325 participants to evaluate how metformin impacts exercise metabolism. The study found that metformin was associated with higher lactate concentrations during exercise in adults with prediabetes and type 2 diabetes [5].
