A Promise That Feels Just Out of Reach
Imagine living in a small town where the nearest specialist is a day's travel away. A mother brings her child with a high fever and no clear answers. Now imagine an AI tool on a phone that helps the local clinic spot the problem in minutes.
That future is closer than many people think. But a new review shows it is not arriving evenly, and the reasons are surprising.
Billions of people live in low- and middle-income countries, often called LMICs. These are places where doctors, nurses, and hospital beds are stretched thin.
Getting tested for cancer, heart disease, or a rare infection can take weeks. Sometimes it never happens at all.
AI has been pitched as a way to close that gap. It can read scans, sort patient records, and flag problems early. In theory, a small clinic with a laptop could do work that once needed a big hospital.
But theory and reality are not the same thing.
The Old Story About Tech and Health
For years, the common belief was simple. Bring better tools to poorer countries, and care will improve. Just ship the technology and watch it work.
But here's the twist. This new review looked at 60 studies and global policy reports and found that dropping AI into a new country is not like dropping in a new pill.
AI learns from data. And if the data comes from people in wealthy countries, the AI may not understand patients who look, live, or get sick differently.
This doesn't mean AI is failing patients on purpose. It means the tools were never built with them in mind.
How AI "Learns" (And Why That Matters)
Think of an AI model like a new doctor in training. It learns by watching thousands of cases.
If that doctor only trains in one city, they may miss clues common in another. A rash that means one thing in Europe might mean something else in Brazil or India.
The review found that more than 60% of AI models used in LMICs were trained on data that did not reflect local patients. That is like asking a tourist to give directions in a city they have never visited.
It can work. But often, it does not.
The researchers followed a careful method called PRISMA-ScR, which is a way of mapping out what existing studies say. They searched major medical databases from 2000 to 2025.
They focused on three big areas: ethics, rules and regulations, and how AI is actually put to use. In total, 60 studies made the cut, along with health policy reports from groups like the World Health Organization.
What They Found Was Eye-Opening
Only 7.4% of low- and middle-income countries have a national AI strategy. That means most have no clear plan for how AI should be used in hospitals or clinics.
Fewer than 1 in 10 health institutions offer structured AI training for staff. So even when tools arrive, many workers have little guidance on how to use them safely.
The review also highlighted gaps in privacy rules, in how patient data is stored, and in who gets to decide what AI is allowed to do.
Here's Where It Gets Interesting
But there's a bright spot. Case studies from Brazil and India showed that context-sensitive design really does work.
When local doctors, patients, and policymakers help shape the AI from the start, the tools fit better. They respect local customs. They understand local diseases. And people trust them more.
Putting This In The Bigger Picture
Experts in global health have long warned about "digital dependency." That is when poorer countries rely on software built elsewhere, with little say in how it runs.
This review pushes for something different. It calls for local innovation ecosystems, meaning home-grown AI built by and for the communities that use it. The shift is less about downloading tools from abroad and more about growing them at home.
If you or a loved one lives in a country with limited healthcare, AI may not change your visit tomorrow. Most tools are still being tested, shaped, and approved.
But the conversation is moving fast. If your doctor mentions an AI-based test or app, it is fair to ask simple questions. Where was it built? Was it tested on patients like me? Who sees my data?
These are not rude questions. They are smart ones.
The Honest Limitations
This was a scoping review. That means it maps what is known, not what works best in practice.
The authors also note there is limited real-world evidence on how these AI tools perform once deployed. Much of the field is still new.
The review lays out a path forward. It calls for shared global rules, more local training, and stronger privacy laws.
It also calls for patient voices at the table, not just engineers and officials. Real progress may take years, because building trust and training people cannot be rushed.
But the direction is clear. If AI is going to help the people who need it most, it has to be designed with them, not just for them. And that work is only just beginning.
