A trial tested a smart health management platform against standard telephone follow-up for elderly patients leaving the hospital. The study involved 383 people and lasted six months. Researchers found that the platform group showed significantly greater improvements in health-related quality of life compared to the standard care group. This difference was statistically significant with a p-value of 0.026. Participants also reported better physical and mental health scores. Self-management skills improved significantly in the group using the platform, with a p-value less than 0.001. The platform was rated highly for usability by 96.35% of the users who tried it. Additionally, 70.83% of users reported reduced healthcare costs while using the system. No adverse events, serious adverse events, discontinuations, or tolerability issues were reported during the study period. The findings suggest this technology offers a practical framework for integrating into nursing practice for aging populations. Readers should note that this was a cluster randomised controlled trial conducted in a post-discharge care setting. The results indicate a link between using the platform and better outcomes, but more research may be needed to confirm these benefits in other settings.
