Key Takeaway
Consider acupuncture as a non-pharmacologic option for chronic low back pain, but note limited evidence and protocol variability.

This clinical practice guideline focuses on acupuncture for adults with chronic non-specific low back pain (CNLBP). The authors synthesized existing evidence and used the GRADE approach for evidence grading along with a Delphi process for expert consensus to enhance scientific rigor.

The guideline states that acupuncture can effectively relieve pain, improve function, and avoid adverse drug reactions. However, no pooled effect sizes, p-values, or confidence intervals are reported. The primary outcome and comparator are not specified.

Limitations acknowledged by the authors include that existing guidelines for low back pain have a broad scope, the evidence assessment of acupuncture for CNLBP is not comprehensive, and there is a lack of consensus on diverse acupuncture treatment protocols. Safety data such as adverse events, serious adverse events, and discontinuations were not reported.

The guideline aims to provide clinicians with standardized and operable acupuncture treatment plans and promote standardization of acupuncture in CNLBP management. Clinicians should interpret these recommendations cautiously given the limitations in evidence quality and protocol variability.

How this fits prior evidence

This guideline on acupuncture for CNLBP adds a non-pharmacologic option to prior coverage. It complements findings that shooting games and equestrian training reduce pain intensity in CLBP, and that lumbosacral orthoses may improve pain and disability in select patients. It also contrasts with the meta-analysis on bariatric surgery for morbidly obese patients, which targets a different population. The guideline's qualitative conclusion aligns with the yoga meta-analysis in supporting non-pharmacologic interventions, though both note limited evidence.

Clinical experts have developed new guidelines to help doctors treat adults with chronic non-specific low back pain. These guidelines aim to create a standard way for healthcare providers to use acupuncture as a treatment option. The goal is to provide more consistent care for patients dealing with long-term back discomfort.

The guideline suggests that acupuncture can effectively relieve pain and improve physical function. Because it is a non-drug treatment, it also avoids the risk of adverse drug reactions. This makes it a potential option for those looking for ways to manage their symptoms without medication.

It is important to note that these guidelines are still evolving. The current evidence for acupuncture in treating low back pain is not yet fully comprehensive, and there is no single agreed-upon method for how the treatment should be performed. Patients should talk to their doctor to see if this approach fits their specific needs.

What this means for you:
Acupuncture may help manage chronic low back pain and improve function without the risk of drug side effects.

Common questions

Can acupuncture help with my chronic back pain?

The clinical guidelines indicate that acupuncture can effectively relieve pain and improve function for adults with chronic non-specific low back pain. It is intended to provide a standardized treatment plan for clinicians to use when managing these symptoms in their patients.

Are there side effects to using acupuncture for back pain?

One of the benefits noted in the guidelines is that acupuncture can help manage pain while avoiding adverse drug reactions. Because it is not a medication, it provides an alternative for those concerned about the side effects of traditional drugs.

Is there a standard way to perform acupuncture for back pain?

Currently, there is no consensus on specific treatment protocols for acupuncture. Because the evidence is not yet fully comprehensive, patients should consult with their healthcare provider to determine the best approach for their specific condition.

BackgroundChronic non-specific low back pain (CNLBP) is a common musculoskeletal disease that troubles adults worldwide. Adverse reactions associated with drug therapy are unavoidable, so non-pharmacological treatments have gained attention. Acupuncture can effectively relieves pain, improve function, and avoid adverse drug reactions. However, existing guidelines for low back pain have a broad scope, and the evidence assessment of acupuncture for CNLBP is not comprehensive. In addition, there are various types of acupuncture, and there is still a lack of consensus on diverse acupuncture treatment protocols. Therefore, the formulation of this guideline will fill the gap and meet the clinical needs.MethodsThe main steps for the formulation of this guideline will include: (1) Establishing a guideline development group; (2) Managing conflicts of interest; (3) Researching and identifying clinical questions; (4) Searching and assessing evidence; (5) Assessing and grading the quality of evidence; (6) Developing decision tables for recommendations; (7) Reaching consensus and making decisions on recommendations; (8) Formulating treatment protocols; (9) Drafting the guideline document; (10) Conducting peer review.DiscussionThis guideline is the first to focus its scope on CNLBP. It systematically synthesizes research evidence on acupuncture treatment, combines experts’ clinical experience with patients’ preferences, and develops clinically applicable recommendations and treatment plans. We incorporated the GRADE approach for evidence grading and a Delphi process for expert consensus. This process enhanced the scientific rigor and practical applicability of the recommendations. This guideline aims to provide clinicians with standardized and operable acupuncture treatment plans, and promote the standardization of acupuncture in the management of CNLBP.Clinical trial registrationhttp://www.guidelines-registry.cn/, identifier [PREPARE-2024CN071].
