American society of anesthesiologists classification 1-3 patients
>20 years old

Exclusion Criteria

patients refuse
dementia patients, cooperation difficult patients
anaphylaxis of dexmedetomidine, ketamine, local anesthetics
heart problem (bradycardia, atrioventricular block)
