Can home exercise help reduce fatigue and anxiety in Colorectal Cancer patients?

limited confidence  ·  Last reviewed June 11, 2026

Fatigue and anxiety are common and distressing symptoms for people with colorectal cancer (CRC). Home-based exercise — physical activity done at home without a gym or clinic — has been studied as a possible way to ease these symptoms. A recent meta-analysis found that home exercise may reduce both fatigue and anxiety, but the evidence is considered low certainty, meaning the results should be interpreted cautiously. Other studies also show benefits for fatigue and quality of life, though effects on anxiety are less consistent.

What the research says

A 2026 meta-analysis of 12 randomized controlled trials involving 802 CRC patients found that home-based exercise significantly reduced anxiety and fatigue compared to usual care 3. However, the certainty of this evidence was graded as low, meaning future research could change these findings 3. Another meta-analysis of home-based physical activity in cancer survivors (mostly breast cancer) found a small but significant effect on fatigue immediately after the intervention, but only 4% of participants had CRC, so results may not fully apply 9. A 2025 feasibility study of a supervised home-based exercise program in 23 CRC patients after surgery reported significant reductions in fatigue and improvements in role and cognitive function, but no significant changes in anxiety or depression 11. A 2024 study protocol for a resistance exercise program in CRC patients undergoing chemotherapy includes home exercises and will measure anxiety and fatigue as secondary outcomes, but results are not yet available 10. Overall, the evidence suggests home exercise may help with fatigue and possibly anxiety, but the low certainty and mixed results mean patients should discuss this with their doctor.

What to ask your doctor

  • Would a home-based exercise program be safe and appropriate for my current treatment stage?
  • What type and intensity of home exercise might help with my fatigue or anxiety?
  • Are there any local resources or referrals for supervised home exercise programs?
  • How can I monitor my symptoms to see if exercise is helping?
  • Should I combine exercise with other strategies for managing fatigue and anxiety?
