Is robotic surgery safe for treating colorectal cancer and liver metastases together?

moderate confidence  ·  Last reviewed July 2, 2026

For patients with colorectal cancer that has spread to the liver, surgery to remove both the colon tumor and liver tumors at the same time (simultaneous resection) is an option. Robotic surgery, using systems like the Da Vinci Xi, offers a minimally invasive approach. Early evidence suggests this technique is safe and feasible for carefully selected patients, though it requires specialized expertise.

What the research says

A systematic review of 9 studies including 29 patients who underwent simultaneous robotic-assisted resection for colorectal cancer and liver metastases found no conversions to open surgery and no perioperative deaths 10. The median operative time was 399.5 minutes, and median blood loss was 274 ml 10. Overall complication rate was 38%, with major complications (Clavien-Dindo grade 3 or higher) occurring in only 7% of patients 10. A separate review focusing on the Da Vinci Xi platform analyzed 78 patients and reported a median operative time of 399 minutes and mean blood loss of 180 ml 11. Post-operative complications occurred in 71.7% of patients, but most (41%) were minor (grade 1 or 2), and no 30-day mortality was reported 11. A case report of a robotic ALPPS procedure (a two-stage liver surgery) for colorectal liver metastases also described successful outcomes with no major complications and short hospital stays 9. These studies indicate that robotic simultaneous resection is technically feasible and safe in experienced hands, though data are still limited to small, non-randomized studies.

What to ask your doctor

  • Is a simultaneous robotic resection a good option for my specific tumor locations and liver involvement?
  • What is the surgeon's experience with robotic colorectal and liver surgery?
  • What are the potential risks and benefits of robotic surgery compared to open or laparoscopic approaches for my case?
  • How will my overall health and any previous treatments affect my candidacy for this procedure?
  • What is the expected recovery time and hospital stay with robotic surgery?
