Does adding gemcitabine improve survival for muscle-invasive bladder cancer patients?

high confidence  ·  Last reviewed June 22, 2026

For muscle-invasive bladder cancer (MIBC), the standard treatment has been cisplatin-based chemotherapy before surgery (neoadjuvant chemotherapy) followed by radical cystectomy. Adding gemcitabine to cisplatin is already part of the standard regimen. Recent research shows that adding the immunotherapy drug durvalumab to gemcitabine-cisplatin further improves survival outcomes.

What the research says

The NIAGARA trial, a large phase 3 study, tested adding durvalumab (an immunotherapy) to neoadjuvant gemcitabine-cisplatin chemotherapy in cisplatin-eligible MIBC patients. The durvalumab group received gemcitabine-cisplatin plus durvalumab before surgery, then durvalumab alone after surgery. The comparison group received only gemcitabine-cisplatin before surgery. At 24 months, event-free survival was 67.8% with durvalumab vs. 59.8% without, and overall survival was 82.2% vs. 75.2% 9. A cost-effectiveness analysis of the same trial confirmed that adding durvalumab to gemcitabine-cisplatin improved overall survival 10. Another review notes that cisplatin-based neoadjuvant chemotherapy (which includes gemcitabine) is the standard of care for MIBC, and that adding immune checkpoint inhibitors like durvalumab is a new advancement 11. While gemcitabine itself is a key part of the standard regimen, the question of whether adding gemcitabine improves survival is answered by the fact that gemcitabine-cisplatin is the backbone of neoadjuvant therapy; the NIAGARA trial shows that adding durvalumab to this backbone further improves survival 910.

What to ask your doctor

  • Am I eligible for cisplatin-based chemotherapy, and would gemcitabine-cisplatin be appropriate for my case?
  • Could adding durvalumab to neoadjuvant gemcitabine-cisplatin improve my outcomes?
  • What are the potential side effects of gemcitabine-cisplatin plus durvalumab?
  • Is the NIAGARA trial regimen available at my treatment center?
  • How does my overall health and kidney function affect my treatment options?
