Is cisplatin better than carboplatin for surviving advanced bladder cancer?

For advanced bladder cancer, cisplatin-based chemotherapy has been the standard treatment for years. However, about 30-50% of patients cannot receive cisplatin because of kidney problems, hearing loss, or other health issues 311. Carboplatin is often used as a substitute in these cases. The key question is whether cisplatin offers a survival advantage over carboplatin. Research shows that cisplatin is generally more effective for survival, but carboplatin remains an important option for patients who cannot tolerate cisplatin.

What the research says

A 2026 meta-analysis that combined 11 studies (2 randomized trials and 9 observational studies) with 2,543 bladder cancer patients found that cisplatin-based chemotherapy was associated with better overall survival compared to carboplatin-based regimens 311. The same meta-analysis also reported that cisplatin led to higher cancer-specific survival and objective response rates 311. Another study from 2022 looked at patients with muscle-invasive bladder cancer who received chemotherapy before surgery or radiation. It found that those who got gemcitabine plus cisplatin had a median overall survival of 41 months, compared to 26 months for those who got gemcitabine plus carboplatin 10. This difference was statistically significant, meaning it was unlikely to be due to chance 10. However, the study also noted that patients who received carboplatin were generally older or had worse kidney function, which may have affected the results 10. Importantly, the 2026 meta-analysis confirmed that cisplatin's survival benefit remained even after accounting for these differences 311. It is also worth noting that carboplatin causes less kidney damage, nerve damage, and hearing loss than cisplatin, which is why doctors choose it for patients who cannot handle cisplatin's side effects 311.
