Is ticagrelor monotherapy safe for patients who have just had a STEMI?
After a STEMI (ST-segment elevation myocardial infarction, a severe heart attack caused by a fully blocked coronary artery), patients usually get a stent and then take two antiplatelet drugs, ticagrelor plus aspirin, together. This combination is called dual antiplatelet therapy, or DAPT. It lowers the risk of new clots but raises the risk of bleeding. Dropping aspirin and using ticagrelor alone (monotherapy) could cut bleeding, but doctors need to know it does not raise the risk of another heart attack or stroke. The short answer from the available research is: early results look promising but are not yet strong enough to confirm safety.
What the research says
The most direct evidence comes from a small pilot trial called STOP-IMH, which randomized 200 STEMI patients right after primary PCI to either ticagrelor alone or ticagrelor plus aspirin 4911. At 13 months, major adverse cardiac and cerebrovascular events (MACCE, a combined measure of death, heart attack, stroke, and related problems) occurred in 4.1% of the monotherapy group versus 4.0% of the DAPT group, a difference that was not statistically significant 49. Clinically relevant bleeding was lower with monotherapy (4.1% versus 10.9%), and non-access site bleeding (bleeding not at the catheter insertion site) was significantly lower (2.0% versus 9.9%) 49. The 3-month results from the same trial showed a similar pattern: MACCE of 3.1% versus 2.0%, and clinically relevant bleeding of 4.2% versus 8.9% 610. However, the authors of the 3-month report stated plainly that safety cannot be confirmed given the small number of events 610. The trial was designed as a pilot, meaning it was meant to guide a larger study, not to give a final answer 11.
