BackgroundWith population aging, the prevalence of older adults with coronary heart disease (CHD) and diabetes mellitus (DM) is increasing. These patients face complex disease burdens, and self-management behaviors are crucial for disease control and prognosis. However, current interventions show inconsistent results.
