Skin neglected tropical diseases (NTDs) are a major cause of morbidity in low-resource settings and frequently present with delayed diagnosis. HIV infection may alter clinical manifestations and further complicate timely recognition, yet evidence remains fragmented. To systematically evaluate and quantify the impact of HIV infection on the clinical presentation and diagnostic delay of skin neglected tropical diseases. A systematic review and meta-analysis was conducted following PRISMA 2020 guidelines. PubMed/MEDLINE, Embase, Web of Science, Scopus, and the Cochrane Library were searched. Observational studies comparing HIV-positive and HIV-negative individuals with skin NTDs were included. Risk of bias was assessed using the Joanna Briggs Institute (JBI) Critical Appraisal Tools. Random-effects meta-analyses were performed using Review Manager (RevMan) version 5.4.1. Ten studies met inclusion criteria and meta-analysis of eligible comparative studies showed that HIV infection was associated with significantly higher odds of severe clinical presentation (OR = 4.05, 95% CI 2.65–6.19) and late diagnosis or advanced presentation (OR = 3.91, 95% CI 2.56–5.96), with I² = 0% for both outcomes. Narrative synthesis highlighted atypical and disseminated manifestations and systemic diagnostic barriers among HIV-infected individuals.
