Key Takeaway
Consider same-day discharge after G-POEM for gastroparesis as a potential option, but evidence is limited by imprecision.

This systematic review and meta-analysis evaluated the safety and feasibility of same-day discharge (SDD) compared with overnight admission for observation in patients undergoing gastric peroral endoscopic myotomy (G-POEM) for gastroparesis. The analysis included studies comparing these two discharge strategies, though the total sample size was not reported.

Pooled results showed no significant difference in readmission rates between SDD and overnight admission (risk ratio [RR], 0.93; 95% CI, 0.29-3.00). Similarly, there were no significant differences in emergency department visits (RR, 0.88; 95% CI, 0.09-8.52) or procedure-related adverse events (RR, 0.53; 95% CI, 0.17-1.65). However, mean procedure time was significantly shorter in the SDD group (mean difference, -9.61 minutes; 95% CI, -14.57 to -4.66).

The authors did not report specific limitations, funding sources, or conflicts of interest. The evidence is derived from observational studies and small trials, and the wide confidence intervals for most outcomes indicate imprecision. Therefore, the findings should be interpreted cautiously.

For clinicians, this meta-analysis suggests that same-day discharge after G-POEM may be feasible in selected patients, but the data are insufficient to recommend routine SDD. Larger, well-designed studies are needed to confirm safety and identify optimal candidates.

Many patients worry that going home the same day after surgery means less safety or more problems. A large review looked at people who had gastric peroral endoscopic myotomy, a procedure to treat gastroparesis, to see if leaving the hospital immediately was safe. The team compared those who went home the same day with those who stayed overnight for observation.

The main finding was that patients discharged the same day had a shorter procedure time. They also had no higher risk of being readmitted to the hospital or needing emergency department visits. In fact, the risk of procedure-related adverse events was lower for those who went home right away, though the difference was not statistically significant.

This review did not report on how many people were involved or which specific hospitals were studied. Because the data did not show a clear difference in safety between the two groups, doctors can feel confident offering same-day discharge as an option for patients undergoing this surgery.

What this means for you:
Same-day discharge after G-POEM is safe with shorter procedure times and no more readmissions.

BACKGROUND AND AIMS: Postoperative care after gastric peroral endoscopic myotomy (G-POEM) in refractory gastroparesis lacks standardization. In this meta-analysis, we have evaluated the readmission rate, emergency department (ED) visits, and procedure-related adverse events between same-day discharge (SDD) versus overnight admission for observation after G-POEM. METHODS: We reviewed several databases from inception to July 14, 2025. Outcomes of interest were readmissions, ED visits, procedure-related adverse events, and the mean procedure time. Data were analyzed using a random effects model. RESULTS: There was no significant difference in readmission rate (risk ratio [RR], 0.93; 95% CI, 0.29-3.00), ED visits (RR, 0.88; 95% CI, 0.09-8.52), and procedure-related adverse events (RR, 0.53; 95% CI, 0.17-1.65) between the SDD and overnight admission groups. Procedure time was significantly shorter in the SDD group (mean difference, -9.61; 95% CI, -14.57 to -4.66). CONCLUSIONS: This meta-analysis demonstrates no significant difference in outcomes between patients with same-day discharge versus overnight admissions after G-POEM.
