Same-day discharge after G-POEM for gastroparesis shows no difference in readmission rates
This systematic review and meta-analysis evaluated the safety and feasibility of same-day discharge (SDD) compared with overnight admission for observation in patients undergoing gastric peroral endoscopic myotomy (G-POEM) for gastroparesis. The analysis included studies comparing these two discharge strategies, though the total sample size was not reported.
Pooled results showed no significant difference in readmission rates between SDD and overnight admission (risk ratio [RR], 0.93; 95% CI, 0.29-3.00). Similarly, there were no significant differences in emergency department visits (RR, 0.88; 95% CI, 0.09-8.52) or procedure-related adverse events (RR, 0.53; 95% CI, 0.17-1.65). However, mean procedure time was significantly shorter in the SDD group (mean difference, -9.61 minutes; 95% CI, -14.57 to -4.66).
The authors did not report specific limitations, funding sources, or conflicts of interest. The evidence is derived from observational studies and small trials, and the wide confidence intervals for most outcomes indicate imprecision. Therefore, the findings should be interpreted cautiously.
For clinicians, this meta-analysis suggests that same-day discharge after G-POEM may be feasible in selected patients, but the data are insufficient to recommend routine SDD. Larger, well-designed studies are needed to confirm safety and identify optimal candidates.
