Periarticular muscle assessment and management are core components for managing knee osteoarthritis symptomsWeak muscles in the knee may lead to joint problems

AI-generated summary of the cited source, checked by automated accuracy review. How we work

Key Takeaway
Note that knee-extensor weakness is linked to incident knee osteoarthritis, but evidence for structural modification is less certain.

This mini review synthesizes current evidence regarding periarticular muscle assessment and management, including exercise, weight management, electrical stimulation, and blood flow restriction, for patients with knee osteoarthritis. The review highlights that knee-extensor weakness is associated with incident knee osteoarthritis. However, the authors note that evidence regarding whether muscle-focused care modifies structural worsening is less certain.

Regarding muscle phenotypes, the authors state that these are not validated and there is no evidence of superior outcomes from phenotype-matched care. Findings regarding human inflammatory-biomarker results were noted as heterogeneous, and ferroptosis remains a preclinical hypothesis. The review notes that the current evidence does not support the claim that muscle-focused care modifies structural progression.

Clinical practice should prioritize assessment based on symptoms, patient-priority activities, functional performance, and strength. While exercise and weight management are considered core components of care, the use of electrical stimulation and blood flow restriction requires qualified interpretation. The authors note that muscle-focused care is not yet proven to modify structural progression.

How this fits prior evidence

This review addresses a gap in the management of knee osteoarthritis by focusing on periarticular muscle assessment. It complements existing evidence regarding the role of exercise in reducing pain and the use of digital support to improve physical function. While previous evidence highlighted a growing focus on inflammatory signaling, this review notes that human inflammatory-biomarker findings remain heterogeneous.

Living with knee pain can make daily activities feel like a constant struggle. New research highlights why the muscles surrounding the joint are just as important as the joint itself. Specifically, weakness in the muscles that extend the knee is linked to the development of knee osteoarthritis.

While muscle-focused care is a core part of management, the evidence is still mixed on some points. For example, researchers have not yet proven that targeting specific muscle types leads to better outcomes than standard care. Additionally, there is currently less certainty regarding whether these muscle-focused treatments can stop the physical worsening of the joint structure.

Doctors suggest starting treatment by looking at a patient's symptoms and the activities they value most. While exercise and weight management are standard tools, other methods like electrical stimulation or blood flow restriction require expert interpretation. The goal is to focus on functional performance and strength to help patients move more comfortably.

What this means for you:
Weak muscles that extend the knee are linked to the development of knee osteoarthritis.

Common questions

How does muscle strength affect my knee health?

Weakness in the muscles that extend the knee is linked to the development of knee osteoarthritis. Because these muscles support the joint, maintaining their strength is a core part of managing the condition and improving how your body functions during daily activities.

Can muscle-focused care stop my joint from getting worse?

The evidence is currently less certain regarding whether muscle-focused care can stop the structural worsening of the joint. While muscle management is a key part of care, it has not been proven to modify the physical progression of the joint structure.

What are the best ways to manage my knee muscles?

Exercise and weight management are considered core parts of care. Other methods, such as electrical stimulation and blood flow restriction, are also used but require a qualified professional to interpret and provide guidance.

Periarticular muscle abnormalities are common in knee osteoarthritis (KOA), but atrophy, weakness, adverse muscle composition, and impaired activation are not interchangeable. This mini review distinguishes their clinical relevance from their present capacity to guide treatment. Longitudinal evidence links knee-extensor weakness to incident KOA; evidence for structural worsening is less certain. Muscle imaging and activation measures provide additional information, but neither validated muscle phenotypes nor superior outcomes from phenotype-matched care have been established. A decision-linked assessment should begin with symptoms, patient-priority activities, functional performance, and strength. Imaging, laboratory activation testing, and sarcopenia assessment are added selectively. Exercise and, for people with overweight or obesity, weight management remain core care. Resistance, aerobic, neuromuscular, mind-body, and task-specific strategies can be adapted to measured impairments and patient preferences without requiring a mechanistic label. Pain and effusion deserve attention when activation is limited; electrical stimulation and blood flow restriction require qualified interpretation of their evidence. Nutritional assessment and monitoring of strength are relevant during intentional weight reduction as well as in patients at risk of sarcopenia. Human inflammatory-biomarker findings remain heterogeneous, and ferroptosis is a preclinical hypothesis. The proposed framework organizes assessment and treatment discussions; it is not a validated classification or evidence that muscle-focused care modifies structural progression.
