Key Takeaway
Note the expert consensus recommendations for managing cardiometabolic risk in patients on androgen deprivation therapy.

This expert consensus guideline provides a framework for managing cardiometabolic risk in patients with prostate cancer who are undergoing androgen deprivation therapy (ADT) or androgen receptor-targeted therapy. The consensus was developed by a panel of 12 clinicians using a modified Delphi method to establish standardized practices for assessment, monitoring, and tailored treatment plans.

The panel achieved consensus on 12 out of 13 statements regarding the management of these patients. The scope of the guidelines includes the assessment of lifestyle risk factors, cardiovascular disease risk assessment, and the implementation of tailored treatment plans. These recommendations aim to balance cancer treatment with the management of cardiovascular risks associated with androgen-related therapies.

A specific limitation noted by the authors is the rejection of routine baseline electrocardiograms. This decision was based on limited evidence, logistical and interpretative challenges, and the potential for delaying necessary cancer treatment. The recommendations provide a structured approach for clinicians to manage the complex interplay between prostate cancer and cardiovascular health in this patient population.

How this fits prior evidence

This guideline addresses a gap in the management of cardiometabolic risks for patients on androgen deprivation therapy. While previous evidence noted that enzalutamide plus ADT improves survival in patients with APIC-negative metastatic prostate cancer, these new guidelines specifically focus on the cardiovascular monitoring and management of patients undergoing such therapies. The consensus provides a structured framework for managing the side effects of androgen-related treatments.

A group of 12 clinicians developed a set of guidelines to help manage heart and metabolic health in patients with prostate cancer. These patients are often undergoing androgen deprivation therapy or androgen receptor-targeted therapy, which can impact cardiovascular health. The goal is to provide a clear plan for monitoring and treating these risks.

The experts reached a consensus on 12 out of 13 key statements regarding patient care. These statements focus on assessing lifestyle factors, monitoring heart health, and creating personalized treatment plans. The guidelines aim to balance the needs of the patient's cancer treatment with the need to manage long-term heart health.

One specific point of caution is that the experts did not recommend routine heart tests like electrocardiograms at the start of treatment. They felt these tests could be difficult to interpret or might delay necessary cancer treatments. These guidelines are based on expert consensus rather than a large clinical trial, so they serve as a framework for doctors to improve care for their patients.

What this means for you:
Experts developed new guidelines to help manage heart and metabolic risks for patients on prostate cancer therapies.

Common questions

Who do these new guidelines help?

These guidelines are specifically designed for patients with prostate cancer who are undergoing androgen deprivation therapy or androgen receptor-targeted therapy. They help doctors manage the heart and metabolic risks that can occur during these specific types of cancer treatments.

What specific risks are being managed?

The guidelines focus on cardiometabolic risks. This includes assessing lifestyle risk factors and monitoring heart health to create tailored treatment plans for patients. The goal is to balance cancer treatment with the management of cardiovascular health.

Are heart tests like EKGs recommended for everyone?

Routine baseline electrocardiograms were not recommended in these guidelines. The experts noted that these tests can be difficult to interpret and might cause delays in starting necessary cancer treatments. You should discuss your specific monitoring plan with your doctor.

Prostate cancer is one of the most diagnosed malignancies among men globally, with androgen deprivation therapy (ADT) and androgen receptor-targeted therapy remaining central to the management of advanced disease. Despite their proven efficacy in controlling tumour progression, these therapies are associated with significant cardiovascular and metabolic risks. The authors aimed to develop a set of evidence-based recommendations for the assessment, monitoring, and management of cardiometabolic risks associated with ADT and androgen receptor-targeted therapy in patients with prostate cancer. A panel of 12 clinicians from Malaysia and Singapore, with expertise in prostate cancer and cardiovascular disease management, developed consensus statements using a modified Delphi method. Statements were informed by a literature review and refined through structured discussions and anonymous voting until consensus was achieved. A total of 13 statements were developed, of which 12 achieved consensus. Key recommendations include conducting a baseline evaluation of lifestyle risk factors and comorbidities, implementing a simplified cardiovascular disease risk assessment checklist, and tailoring treatment plans and structured management protocols according to individual cardiovascular disease risk categories. The panel also recommended referring high-risk or symptomatic patients for appropriate care (if not already on follow-up) and initiating cardiometabolic monitoring post-treatment with frequency adjusted based on patient-specific risk profiles. Additional endorsed strategies included incorporating lifestyle modification counselling during follow-up, applying the modified ABCDE Cardio-Oncology framework, and involving multidisciplinary teams in managing patients with elevated cardiovascular risk. The rejected statement, which proposed routine baseline electrocardiograms for all patients, lacked support owing to limited evidence, logistical and interpretative challenges, and concerns over delaying cancer treatment. This consensus document offers practical, evidence-based guidance for minimising cardiometabolic risks in patients with prostate cancer on ADT or androgen receptor-targeted therapy. These recommendations support individualised care, balancing oncological efficacy with cardiovascular safety to improve patient outcomes.
