A new analysis of observational studies suggests that mechanical thrombectomy, a procedure to remove blood clots, may help children who have had a severe type of stroke called large-vessel occlusion. However, the findings are not definitive, and more research is needed before this treatment can be widely recommended for children.
The analysis combined data from 436 children, 230 of whom received mechanical thrombectomy, while 206 received medical management, which could include intravenous thrombolysis (clot-busting drugs) or conservative care. The main goal was to see if mechanical thrombectomy led to better outcomes, such as improved recovery and function.
When comparing mechanical thrombectomy to medical management as a whole, the analysis found no statistically significant difference in favorable outcomes. However, when the researchers used a different statistical method (Bayesian inference), they found a positive effect, and when they compared mechanical thrombectomy directly to intravenous thrombolysis or conservative treatment, they found that mechanical thrombectomy was associated with better outcomes. For example, the odds of a favorable outcome were about 2.4 times higher with mechanical thrombectomy than with intravenous thrombolysis, and about 2.2 times higher than with conservative treatment.
The analysis did not find enough evidence to determine whether mechanical thrombectomy affects the risk of death, and the authors caution that the findings are hypothesis-generating, not proof. The study was limited by its small sample size and the fact that it was based on observational data, which cannot prove cause and effect.
For now, this analysis suggests that mechanical thrombectomy might be a promising option for children with large-vessel occlusion stroke, but larger, more rigorous studies are needed to confirm these findings and to guide treatment decisions.
