Osteoporosis (OP) is a systemic skeletal disorder characterized by decreased bone mass, impaired bone microarchitecture, and elevated fracture risk. With global population aging, OP has become a major public health burden worldwide. The gut-bone axis, a critical regulatory network connecting gut microbiota (GM) and bone metabolism, has emerged as a frontier in OP pathogenesis and intervention. GM dysbiosis disrupts bone homeostasis through metabolic, endocrine, and immune pathways, including short-chain fatty acids, estrogen/parathyroid hormone signaling, and Th17/Treg balance.
