Prisons, as highly restrictive environments, represent settings where incarcerated individuals are particularly vulnerable to psychological and psychiatric morbidities. Given the substantial fiscal burden associated with conventional psychological interventions, physical activity has emerged as a promising, cost-effective, and readily implementable alternative. However, systematic quantitative evidence regarding the therapeutic efficacy of this intervention remains scarce. Accordingly, this study employed a systematic review and meta-analysis to elucidate the effects of physical activity on anxiety, depression, and stress levels among prison populations. A comprehensive literature search across five authoritative electronic databases yielded 11 eligible studies meeting predefined inclusion and exclusion criteria. Our findings revealed that incarcerated individuals showed improvements in mental health during participation in physical activity interventions, with significant reductions in anxiety (SMD = 0.67, p < 0.001) and depression (SMD = 0.69, p < 0.001). Notably, no statistically significant effect was observed for stress levels (SMD = 1.21, p = 0.154).
