Dengue: what the trials found

Several large phase 3 trials have established the immunogenicity and safety of the CYD tetravalent dengue vaccine (live, attenuated, dengue serotypes 1-4). In a pivotal trial (N=20,869), the vaccine reduced symptomatic virologically confirmed dengue (VCD) cases compared to placebo (273 vs. 380 cases) 2. Another large trial (N=10,275) similarly showed fewer VCD cases after dose 1 (276 vs. 302) and dose 2 (205 vs. 238) 7. Seropositivity rates against all four serotypes were high post-vaccination, with antibody titers ≥10 1/dilution achieved in 72.8-94.9% of participants across serotypes 2 and 52.0-94.0% in another study 7.

Safety data from phase 3 trials indicate that the CYD vaccine is generally well tolerated. In a study of 400 participants, solicited local adverse events occurred in 34.3-56.2% and systemic events in 58.6-66.6% after vaccination, with most being mild to moderate 4. Unsolicited adverse events were reported in 25.0-30.0% 4. A phase 2 trial (N=1,198) reported solicited injection-site and systemic reactions in 53.9% and 66.7% of participants, respectively, with low rates of severe reactions (1.0-2.0%) 14. Another phase 2 study (N=4,002) found solicited injection-site reactions in 57.7% and systemic reactions in 32.2% 15. Concomitant administration with yellow fever vaccine did not impair immune responses, with seroconversion rates of 98.7-100% 6.

Immunogenicity data from phase 2 trials support robust antibody responses. A study of the tetravalent CYD vaccine (N=90) demonstrated seropositivity against all serotypes 12. Another trial (N=86) using a live attenuated tetravalent vaccine reported low rates of grade 3 solicited symptoms (0-2 subjects) and unsolicited adverse events (6 subjects) 19. A phase 2 study of a tetravalent live attenuated vaccine (N=51) showed seronegative neutralizing antibody titers after dose 2 ranging from 25.0% to 100% across serotypes 22.
