Hypoparathyroidism: what the trials found

Several recombinant human parathyroid hormone (rhPTH) formulations have demonstrated efficacy in managing hypoparathyroidism symptoms and clinical markers. rhPTH(1-84) was associated with significant improvements in symptom scores, including the Hypoparathyroidism Symptom Diary (HypoPT-SD) and physical component summary scores 3, while also showing a 5.5% increase in bone mineral density (BMD) via DXA 7. Another PTH formulation showed a 2.9% to 0.95% change in BMD over 24 months 8.

TransCon PTH demonstrated significant improvements in physical and cognitive domain scores on the HPES scale compared to placebo 6, though its primary and secondary efficacy endpoints did not reach statistical significance in another trial 12. Additionally, NPSP558 was associated with significant reductions in required oral calcium and active vitamin D dosages over time [5, 11].

Placebo groups showed significantly lower rates of meeting triple efficacy endpoints compared to treatment groups, which also demonstrated a significant reduction in daily calcium requirements and an increased proportion of patients achieving independence from high-dose vitamin D and oral calcium 9.
