Mania: what the trials found

Lithium treatment for mania was associated with specific Sedation Scores ranging from 0.4 to 0.6 and YMRS scores between 14.3 and 21.4 1. Quetiapine demonstrated significant reductions in YMRS total scores, including a reduction of -20.72 at the end of treatment (Day 21) for a 600mg dose 3, and a reduction of -14.5 in bipolar symptoms as measured by Y-MRS 5.

Cariprazine showed statistically significant improvements in mania symptoms at week 3. Specifically, it resulted in YMRS score reductions ranging from -12.5 to -19.6 (p<0.001; p=0.0004) and CGI-S score reductions of -1.3 to -1.9 (p<0.001; p=0.0027) [6, 7].

Lamotrigine was evaluated in the context of mania and alcohol use, showing a reported abstinence rate of approximately 78-80% and a reduction in heavy drinking days to between 8.1% and 11.6% [2, 4].

Recent results — preliminary, needs further review

  • Omega 3 showed a change from baseline in depression symptom severity at 12 weeks (17.7 to 18), but this finding is not yet corroborated.
  • Pentazocine was evaluated on the MACS and YMRS scales, but results did not reach statistical significance (p=0.22; p=0.83).
  • Cognitive Interpersonal Therapy showed a significant change in Internal State Scale scores (p<0.05), though changes in MADRS and BRMS scales were not statistically significant.

For the clinician treating this condition

  • Quetiapine and Cariprazine are supported by evidence for reducing YMRS scores in patients with mania.
  • Cariprazine demonstrated consistent, statistically significant improvements in both YMRS and CGI-S scores at the 3-week mark.
  • Lithium is established as an intervention associated with specific sedation and mania rating scales.
