What the trials found For clinicians

Uremic pruritus: what the trials found

CR845 0.5 mcg/kg demonstrated significant reductions in itch intensity and improvements in quality of life for patients with uremic pruritus. Specifically, a majority of patients achieved an improvement of ≥3 points on the Weekly Mean Daily 24-hour Worst Itching Intensity NRS at week 12 [1, 2]. Additionally, CR845 showed significant reductions in the mean NRS score at week 8 [-3.8; -2.8; -3.2; -1.9, p<0.001] and corresponding improvements in Total Skindex-10 scores [-18.7; -15.5; -15.1; -8.2, p<0.001] 4.

MR13A9 showed a statistically significant reduction in mean NRS scores at week 4 (-2.06; -1.09, p<0.001) 7, although results for MR13A9 at week 8 did not reach statistical significance (p=0.770) 10. Difelikefalin Injection also demonstrated a significant reduction in the Weekly Mean of the Daily 24-hour WI-NRS score at week 4 (-2.09; -1.27, p=0.0003), with varying percentages of patients achieving ≥3 or ≥4 point improvements 8.

Several Phase 3 trials evaluated nalbuphine HCl ER for uremic pruritus [3, 9], and other studies investigated CR845 [5, 6] without reporting specific outcome values in the provided data.

Recent results — preliminary, needs further review

  • Phototherapy (Narrow band UVB and long wave UVA) showed a reduction in Visual Analogue Scale (VAS) scores for pruritus (-3.38; -3.53, p<0.05), though this finding is not yet corroborated.

For the clinician treating this condition

  • CR845 0.5 mcg/kg is associated with significant reductions in itch intensity (NRS) and improvements in patient-reported quality of life (Skindex-10, 5-D Itch Scale) [1, 2, 4].
  • Difelikefalin Injection demonstrated statistically significant improvements in itching scores at week 4 8.
  • MR13A9 showed significant reduction in mean NRS scores at week 4, but did not reach statistical significance at the week 8 mark [7, 10].
