Mucositis: what the trials found

Pharmacological interventions for mucositis include doxepin hydrochloride, which demonstrated significant reductions in total pain of the mouth and throat (-9.1; -4.7, p<0.001) while also showing statistically significant improvements in taste (p=0.0018) and a reduction in stinging or burning sensations from oral rinses (p=0.001) 1. Gabapentin was shown to significantly reduce pain associated with radiation-induced mucositis on the VHNSS scale (p=0.004) and resulted in lower scores for general systemic symptoms (p<0.001) without increasing the frequency of Grade 3 or 4 adverse events 2.

Clinical trials comparing placebos to other interventions showed that placebo groups experienced significantly higher rates of severe (WHO Grade 3 or 4) oral mucositis, shorter times to onset of severe mucositis, and longer durations of severe mucositis compared to treatment arms 4. Low Level Laser Therapy was evaluated in patients receiving high cumulative radiation doses; results indicated specific counts for participants experiencing any grade, WHO Grade 3-4, or CTCAE Grade 3-5 severe oral mucositis [14, 15].

Ketamine was evaluated for pain management in this condition; however, one trial reported no statistically significant change in pain scores (p=0.3198) or duration of analgesic effect (p=0.3522) 12, while another noted a mean change in pain scores of 2 8. Other interventions such as fentanyl sublingual spray 5 and supersaturated calcium phosphate rinse 3 were also evaluated in established trials.
