Scalp psoriasis: what the trials found

Risankizumab demonstrated significant efficacy for scalp psoriasis, with 60.8% of participants achieving a Scalp Investigator Global Assessment (IGA) of 0 or 1 at Week 16 (p<0.0001) 1. Roflumilast Foam 0.3% also showed statistically significant improvements, including a 66.4% success rate in Scalp Investigator Global Assessment (S-IGA) at Week 8 (p<0.0001) and significant improvement in the Scalp Itch-Numeric Rating Score (SI-NRS) at Week 2 (p=0.0005) 3.

Clobetasol propionate was evaluated in multiple formulations; a spray formulation showed clinical activity in reducing scaling, erythema, and plaque elevation by Week 4 2, while a shampoo formulation was compared against other corticosteroids where 21.8% of subjects preferred the clobetasol propionate shampoo over comparators (p=0.0003) 9. Xamiol gel demonstrated a success rate (Total Sign Score ≤1) of 64.2% at Week 4 8.

The topical suspension/gel LEO 80185 showed a 54.8% rate of controlled disease (Clear or Almost Clear) by Week 4, with low rates of adverse drug reactions ranging from 3.2% to 6.4% [11, 12]. Other interventions such as DSXS topical and DSXS did not show statistically significant differences in clinical success compared to their respective controls across various severity levels [6, 7].

Recent results — preliminary, needs further review

  • Risankizumab is a newly established treatment showing high rates of Scalp IGA 0 or 1 (60.8%) at Week 16 1.

For the clinician treating this condition

  • Risankizumab and Roflumilast Foam 0.3% are supported by significant data for achieving clear or almost clear scalp outcomes in clinical trials.
  • Clobetasol propionate remains a viable option with evidence of efficacy in reducing physical signs like scaling and erythema.
  • Xamiol gel and LEO 80185 show consistent results in disease control with low reported rates of adverse drug reactions.
