Hodgkin lymphoma: what the trials found

New evidence regarding fludarabine indicates significant improvements in disease-free survival (70.6%, p=0.002) and a reported overall survival of 73.3% 19. Other trials evaluating fludarabine established its use in various contexts, including studies on overall survival 14, non-relapse mortality 14, and relapse/progression rates 14.

Clinical evaluations of other agents include cyclosporine for managing acute GVHD 1 and general use 7, as well as sirolimus 13. Treatment with doxorubicin hydrochloride has also been established 10. Additionally, a supersaturated calcium phosphate rinse was evaluated for the duration of severe oral mucositis (WHO Grade 3 or 4), showing equal durations in study arms (4.5 days) 12.

In the context of bone marrow transplantation, haploidentical bone marrow transplant showed a statistically significant improvement in neutrophil recovery (98.8% vs 94.9%, p=0.046), though progression-free survival did not reach statistical significance (p=0.4092) 11. Other agents such as bleomycin sulfate were evaluated for non-hematologic toxicity [8, 9], and brentuximab vedotin was assessed regarding patient-reported outcomes 6 and subsequent radiation requirements 6.

Recent results — preliminary, needs further review

  • * Bendamustine is currently under investigation in a small cohort (N=3) 16.

For the clinician treating this condition

  • Fludarabine is associated with statistically significant improvements in disease-free survival 19.
  • Haploidentical bone marrow transplant significantly improves the rate of neutrophil recovery compared to alternative protocols 11.
  • Supersaturated calcium phosphate rinse was evaluated for managing the duration of severe oral mucositis 12.
