What the trials found For clinicians

Hypertrophic Cardiomyopathy: what the trials found

Mavacamten has been evaluated in Phase 2 and Phase 1 clinical trials for the management of hypertrophic cardiomyopathy [4, 7]. In a Phase 2 study, mavacamten was associated with mean changes from baseline in KCCQ PRO scores ranging from 5.809 to 18.600 4.

Spironolactone showed no statistically significant differences compared to placebo regarding serum markers of collagen turnover (p=1.0), peak oxygen consumption during exercise (p=0.7), or New York Heart Association functional class symptoms (p=0.8) over a one-year period 1.

Diltiazem was evaluated for its impact on diastolic function, where no significant difference was found in the decline of Global Early Myocardial Relaxation (E') velocity (p=0.75). Diltiazem also demonstrated acceptable safety and tolerability (p=0.99) and did not significantly impact heart rate (p>=0.06) 3. Ranolazine has also been studied in this patient population 2.

Recent results — preliminary, needs further review

  • Losartan showed a statistically significant reduction in the extent of left ventricular fibrosis at 1 year (-23% vs 31%, p=0.02), though changes in left ventricular mass did not reach statistical significance (p=0.06) 6.
  • LCZ696 did not show a statistically significant change from baseline in peak VO2 as measured by CPET (p=0.5506) 5.
  • N-acetylcysteine showed a 92% compliance rate in a Phase 1 study 8.

For the clinician treating this condition

  • Mavacamten is established in early-phase trials with observed improvements in KCCQ PRO scores [4, 7].
  • Spironolactone did not demonstrate significant changes in collagen turnover markers or functional capacity over a one-year period 1.
  • Diltiazem was found to be well-tolerated and did not significantly alter heart rate or diastolic function (E' velocity) 3.
