Inguinal Hernia: what the trials found

Clinical evidence for inguinal hernia management includes various pharmacological and procedural interventions. The use of regional anesthesia was associated with specific cognitive performance scores, including Full Scale IQ (100.5; 100.1), Sentence Repetition (9.7; 9.7), and Auditory Attention (9.0; 9.3) 3. Additionally, a combination of Bupivacaine, Dexmedetomidine, Caffeine, and Tylenol was associated with hospital discharge within 2 or 1.6 days post-surgery 7.

Analgesic interventions show varied outcomes in pain management. Acetaminophen IV solution resulted in PACU Visual Analogue scores ranging from 1.45 to 2.88 5, while 0.25% Bupivacaine was associated with rest pain scores of 3.75 and 4.01, and movement pain scores of 4.93 and 5.46 11. Pregabalin showed no statistically significant difference in pain scores for both sitting (p=0.4471) and walking (p=0.6896) movements 12. A Bupivacaine Collagen Sponge was associated with specific Visual Analogue Pain Scale scores during rest and aggravated movement, as well as a reduction in opioid rescue analgesia over 24 to 72 hours [14, 15].

Surgical materials and techniques have been evaluated for efficacy. Repair with Ultrapro showed significant changes from baseline in Activities Assessment Scale at 3, 6, and 12 months 6. The use of Parietex Progrip was associated with specific VAS scores over one year, though chronic pain (defined as >3 months) was noted in both study arms 4. Other materials such as AbsorbaTack 8 and mesh fixation 10 were evaluated; however, the latter did not show a statistically significant difference in postoperative pain proportions (p=0.2) or seroma formation (p=0.56). Prophylactic antibiotics (Gentamicin and cefazolin) showed no statistically significant difference in infection rates (p=0.198) 9.
