What the trials found For clinicians

Left ventricular dysfunction: what the trials found

Diagnostic imaging using AdreView (Iobenguane I 123 Injection) demonstrated reproducible myocardial uptake on planar imaging at both 3 hours and 15 minutes post-injection 1.

In clinical management, Captopril was evaluated for its impact on cardiovascular death, hospitalization, and heart failure symptoms over a 9-month period 3. Ivabradine did not show statistically significant differences in primary composite endpoints, cardiovascular death, or hospitalization for acute myocardial infarction 5.

Stem cell therapies showed mixed results regarding cardiac function. One trial of adult stem cells reported no significant changes in global or regional left ventricular function (infarct and border zones) 8, nor did it show significant changes in VO2max, LVESV, or reversible defect size 9. However, a different study of adult stem cells showed statistically significant improvements in global and regional (both infarct and border zone) left ventricular function 10.

Eculizumab was evaluated in the context of heart transplantation, where 18 patients survived at 12 months with zero instances of hemodynamic compromise at 6 months post-transplant 4.

Recent results — preliminary, needs further review

  • Autologous human mesenchymal cells (hMSCs) were studied for effects on serial troponin and creatine kinase values, as well as changes in the MLHF questionnaire score 6.
  • Riociguat was evaluated for changes in pulmonary artery mean pressure and E/A ratios, with results not reaching statistical significance 7.
  • Wearable Cardioverter Defibrillators were assessed in a large cohort 11.
  • HOT-CRT and CRT-D devices were investigated, with the latter showing an improved clinical composite score in 51.3% of subjects at 12 months [12, 13].
  • MR Spectroscopy was utilized to determine heart failure levels based on signal-to-noise ratios 16.

For the clinician treating this condition

  • AdreView provides reproducible myocardial uptake measurements for imaging in heart failure patients 1.
  • Adult stem cell therapy shows conflicting evidence regarding the improvement of global and regional left ventricular function; while one study showed no significant change, another demonstrated statistically significant improvements [8, 10].
  • Ivabradine did not demonstrate a statistically significant impact on cardiovascular death or myocardial infarction hospitalization in the studied cohort 5.
